  During the enrollment procedure parents must complete all the following forms:


  • Parent notification additional children in care
  • Affidavit regarding liability insurance for family child care home
  • Consent for emergency medical treatment
  • Nebulizer care consent / verification
  • Identification and emergency information
  • Notification of parents rights
  • Caregiver background check process
  • Family child care consumer awareness information
  • Acknowledgement of receipt of licensing reports
